Rationale: Chronic Obstructive Pulmonary Disease (COPD) is associated with body composition alterations such as sarcopenia. The objective was to determine the association between pulmonary function and sarcopenia in COPD patients.

Methods: A Cross-sectional study. Patients with COPD. Subjects with exacerbations within <3 months were excluded. Body composition was assessed by bioelectrical impedance analysis. Sarcopenia was defined according to EWGSOP2 criteria: low muscle strength, confirmed by low muscle quantity or quality. Pulmonary function was assessed by spirometry.

Results: A total of 513 patients were evaluated, 16.2% had sarcopenia. Mean age was 70.58 ± 10.88; 53.80% were men. Sarcopenia subjects had lower diabetes (20.48% vs 31.87%, p=0.038), obstructive sleep apnea (4.84% vs 18.60%, p=0.008), FEV1 (1.16 ± 0.45 vs 1.36 ± 0.67, p=0.037), FVC (2.12 ± 0.86 vs 2.41 ± 0.87, p=0.020), DLCO (10.69 ± 5.29 vs 16.97 ± 11.90, p=0.035), higher impedance index (0.86 ± 0.06 vs 0.83 ± 0.10, p=0.023), and % extracellular water (26.23 ± 7.76 vs 23.83 ± 5.64, p=0.003). In the multivariate model, subjects with sarcopenia had 0.17 L lower FEV1 (β: -0.17, 95% CI: -0.37 to -0.03) and 0.26 L lower FVC (β: -0.26, 95% CI: -0.53 to -0.04) compared with subjects without sarcopenia.

Conclusion: Sarcopenia in COPD patients is associated with reduced pulmonary function, highlighting the importance of early detection for timely intervention.
