Rationale: Given the lack of nutritional strategies for cancer cachexia, this randomized, placebo-controlled trial evaluated the impact of grape seed flour vs placebo on cytokines in colorectal cancer cachexia in treatment-naïve patients.
Methods: The study was conducted in Santos, São Paulo, Brazil (two hospitals). Samples of rectus abdominis muscle and tumor tissue were collected during surgical resection, after four weeks of supplementation (8g/day of grape seed flour or starch) and cytokines were analyzed using the Luminex platform. Data were analyzed jointly Exploratory Factor Analysis (EFA), using minimum residual extraction with oblimin rotation. The analysis identified two factors for each tissue. Tumor tissue factors were labeled as: Pro-inflammatory (IL-1β, IL-6, TNF-β, IL-1RA, IL-1α, IL-8, TNF-α, MCP-1, IFN-γ, and GM-CSF) and Pleiotropic (IL-2, IFN-α2, and IL-10). Muscle tissue factors were labeled as: Pro-inflammatory (MCP-1, IL-1RA, IL-6, IL-8, TNF-β, IL-1β, TNF-α) and Regulatory (IL-10, IL-2, IFN-α2, IFN-γ, and GM-CSF). EFA adequacy was confirmed for tumor tissue (Bartlett’s test p<0.01; KMO≥0.73) and muscle tissue (Bartlett’s test p<0.001; KMO≥0.64).
Results: A total of 25 participants were included (intervention=13; control=12), with a mean age of 65.0±8.4 years, and 56% male. Participants had similar baseline characteristics in terms of height, weight, and BMI (p>0.05). Student’s t-test showed that supplementation did not produce significant changes between groups in tumor cytokine profiles: Pro-inflammatory (p=0.95) and Pleiotropic (p=0.16). Similarly, in muscle tissue, the Pro-inflammatory (p=0.65) and Regulatory (p=0.13) profiles remained unchanged after the intervention.
Conclusion: The clustered cytokine analysis indicates that the intervention did not modify muscle or tumor cytokine profiles after 4 weeks supplementation.
