POSTPRANDIAL INTERSTITIAL GLUCOSE DYNAMICS TRACK METABOLIC RECOVERY IN DIABETIC MINIPIGS

Rationale: Postprandial glucose regulation and glycemic variability are key markers of metabolic homeostasis and may provide early insight into recovery after pancreatic β-cell injury. We evaluated whether continuous glucose monitoring (CGM)-derived interstitial glucose dynamics can track metabolic recovery in streptozotocin (STZ)-induced diabetic minipigs
Methods: Preclinical study of 12 adult minipigs followed for 30 days after STZ administration and monitored using CGM. Diabetes recovery was defined as fasting interstitial glucose at day 30 returning to the pre-induction glycemic range, established by the minimum and maximum baseline values. Based on this definition, animals were classified as recovery or non-recovery. Interstitial glucose and coefficient of variation (CV, %) were compared at baseline, day 15, and day 30 using Mann–Whitney U tests. In animals with complete CGM follow-up, pre- and post-feeding interstitial glucose changes were analyzed
Results: No baseline differences were observed between groups. A total of 12 adult minipigs were included, of which 25% showed metabolic recovery. At day 15, recovery animals showed lower interstitial glucose (53.9 vs 183.6 mg/dL; p=0.052) and lower CV (9.9% vs 21.2%; p=0.033) than non-recovery animals. At day 30, recovery animals maintained lower interstitial glucose (46.0 vs 130.5 mg/dL; p=0.013) and lower CV (15.5% vs 23.7%; p=0.021). Postprandial CGM profiles showed sustained glucose elevation up to 180 min after feeding in non-recovery animals, whereas recovery animals showed smaller glucose excursions and progressive normalization over time

Conclusion: Metabolic recovery after STZ-induced diabetes in minipigs was associated with lower fasting interstitial glucose, improved postprandial glucose regulation, and reduced glycemic variability. CGM may be a useful non-invasive tool for identifying early recovery patterns in translational models of chronic metabolic disease
