SYSTEMIC INFLAMMATION IN URBAN INDIAN ADULTS DRIVEN BY ADIPOSITY: UNTRAMMELLED CONTRIBUTION BEYOND PHYSICAL ACTIVITY AND DIETARY PATTERNS

Rationale: Chronic low-grade inflammation steers cardiometabolic disease, yet independent contribution of adiposity relative to diet and exercise remain poorly quantified in urban Indian population. Study investigated whether adiposity exerts dominant, dose-dependent effect on systemic inflammation, irrespective of lifestyle modifiers.
Methods: Cross-sectional analysis of 1,354 free-living adults(20-50 years) recruited from community of urban Mumbai and Pune(1,067 obese, 287 non-obese; BMI classification). Physical activity evaluated as MET-min/week, frequency of antioxidant and prebiotic-rich food intake assessed with validated semi-quantitative food-frequency questionnaire. Data analyzed- one-way ANOVA evaluating group differences, linear trends across BMI categories(α=0.05).
Results: Inflammatory biomarkers exhibited clear, stepwise dose-response relationship with increasing BMI-

Within non-obese subgroup, moderately active individuals unexpectedly displayed higher hs-CRP and leptin than sedentary individuals, indicating limited anti-inflammatory benefit from moderate activity alone in absence of adiposity control. Frequency of antioxidant or prebiotic-rich food consumption alone also showed no significant correlation with these biomarkers.
Conclusion: Adiposity independently causes systemic inflammation in a dose-dependent manner across BMI levels, persisting regardless of diet and exercise habits. These data underscore that targeted reduction of fat mass, rather than isolated lifestyle modifications, is required to meaningfully attenuate inflammation mediated cardiometabolic risk in high-burden population.
