OUTPATIENTS IN BRAZIL

Rationale: The prevalence of malnutrition in Brazil is high1. One in three outpatients is malnourished2 and in this group,
high-quality oral nutritional supplementation (ONS) is essential1. Aim: to evaluate the quality indicators of ONS in older
outpatients.

Methods: A multicentric, retrospective, observational study with older outpatients in three different health institutions (HI)
in Rio de Janeiro, Belo Horizonte, and Campinas - Brazil. Different protocols were applied: calf circumference
assessment (CC) and MAN short form, ONS compliance, and food intake, all verified by a dietitian.

Results: 72 patients were included, 50% male, 74 +8 years, and the main chronic diseases were hypertension (50%),
diabetes (30%), and stroke (30%). Only two HI applied the MAN-SF protocol, and 90% of the outpatients were
malnourished or at nutritional risk (mean MAN-SF: 9 +2). All the HI measured CC, and 50% were classified as low CC
(mean CC: 31 +4.5) cm. A quality survey identified that only 46% of outpatients with malnutrition/nutritional risk by MANSF
and 44% of outpatients with low CC received ONS. Among outpatients on ONS, 71% were prescribed one bottle/day,
53% had excellent adherence, and 47% consumed half or less than half of the total volume. The main reasons for low
ONS adherence were appetite loss (6.5%), flavour (6.5%), and volume (3%).

Conclusion: The prevalence of malnutrition/nutritional risk in older outpatients in three HI in Brazil is high. Less than half
of older patients with malnutrition received ONS. Therefore, we highlight that nutritional risk assessment protocols and
ONS compliance for older outpatients are essential for improving quality indicators on ONS.
