CHANGES IN BODY COMPOSITION IN WOMEN WITH ELEVATED BMI: IMPACT ON OVARIAN STIMULATION IN IVF

Rationale: Obesity is associated with impaired reproductive outcomes, likely through underlying metabolic dysfunction. Whether targeted nutritional and metabolic optimisation can improve reproductive outcomes in women undergoing assisted reproduction remains unclear.

Methods: This retrospective study included 42 women (BMI >30) undergoing ART after a failed IVF cycle, who participated in a metabolic optimisation program (FERMET). Patients were evaluated before and after the intervention. Body composition was assessed using bioelectrical impedance analysis, and indirect calorimetry was used to define individualized caloric targets.

Results: The FERMET intervention was associated with significant alterations in body composition. The decrease in fat mass and the increase in fat-free mass % (p<0.001) validated a significant metabolic impact. 57% of the women who underwent the FERMET intervention had a live birth. The proportion of women who had a biochemical pregnancy rose from 24% to 71% (p = 0.002). The clinical pregnancy rate rose from 14% to 71% (p < 0.001). However, the changes in ovarian stimulation outcomes (number of oocytes retrieved, number of mature oocytes, number of embryos) were statistically not significant. Correlation analyses indicated the absence of robust linear associations between alterations in body composition and specific IVF laboratory parameters.

Conclusion: Structured metabolic optimisation was associated with significantly improved body composition in obese women undergoing IVF, despite no changes in ovarian response and embryological markers. The findings suggest that metabolic health is a crucial and possibly modifiable factor influencing reproductive outcomes in this group.
