MUSCLE MATTERS: BODY COMPOSITION AND HANDGRIP STRENGTH AS PREDICTORS OF QUALITY OF LIFE IN HPN

Rationale: Sarcopenia and poor nutritional status are common in HPN patients1, yet their relationship with quality of life (QoL) is not well understood. This study aimed to evaluate the prevalence of low muscle mass and poor muscle quality, and to explore their associations with hand grip strength (HGS) and QoL.

Methods: There were 24 adults with CIF included (ethical approval; 21/SC/0316). Fat-free mass index (FFMI; kg/m2) was used to define low muscle mass (<18.2 kg/m2 in men; <15.0 kg/m2 in women)2 and phase angle (PA; degrees) was used to indicate poor muscle quality (<5°).3 HGS was measured using a Takei dynamometer and QoL was evaluated using the SF-36, EQ-5D and HPN-QoL questionnaires.

Results: Demographics are shown in Table 1. The mean PA was 4.7° and mean FFMI was 16.6 kg/m². Low muscle mass was identified in 64% of females and 30% of males. Poor muscle quality was observed in 71% of females and 30% of males. Moderate positive correlations were found between FFMI and HGS (r(22) = 0.645, p = .001), and between PA and HGS (r(22) = 0.486, p = .022). Lower HGS, PA, and FFMI were consistently associated with poorer QoL.

Conclusion: Low muscle mass and poor muscle quality are prevalent in CIF and are strongly associated with poorer QoL. Routine assessment of muscle strength and nutritional parameters may help identify patients at risk of adverse outcomes. Future research should focus on targeted exercise interventions to improve body composition and support QoL.
