Rationale: To assess changes in nutritional status during three months of  dietary treatment in adult patients with (risk of) malnutrition in primary care.   

Methods: Adult patients referred to dietitians with (risk of) malnutrition were included. Nutritional status was assessed by Patient-Generated Subjective Global Assessment (PG-SGA) on day 1, week 2, week 6, and week 12 of dietary treatment. Nutritional status was categorized into: A=well-nourished; B=moderate/suspected malnutrition; C=severely malnourished. Comparisons and directions of change in nutritional status were tested by the Wilcoxon Signed-rank test (significance = p<0.05).

Results: 120 patients (61% female, 72±17 years) completed the PG-SGA at all time points with their dietitians. On day 1 of dietary treatment, 21 (17%) patients were classified into PG-SGA category A, 49 (41%) into category B, and 50 (42%) into category C. Changes in nutritional status during the three months are shown in Table 1.

The change from day 1 to week 2 significantly differed from the change from week 2 to week 6 (p=0.007) and the change from week 6 to week 12 (p=0.006).

Conclusion: With dietary treatment, nutritional status improves significantly in patients after twelve weeks, with the largest improvement within the first two weeks. These findings highlight the early impact of dietary treatment in primary care, supporting timely dietetic referral and follow up to sustain initial gains and promote longer-term improvement.
