BEYOND WEIGHT CHANGE: LONGITUDINAL DECLINE IN MUSCLE, BONE, AND NUTRITIONAL STATUS IN WOMEN WITH INTERSTITIAL LUNG DISEASE

Rationale: Interstitial lung diseases (ILD) are associated with a range of nutritional alterations, including weight loss, changes in body composition, and reduced bone mineral density (BMD). These manifestations are often related to both disease progression and pharmacological treatment, and may negatively affect physical function. This study aimed to describe longitudinal changes in nutritional status and bone health over 38 months of follow-up in women with ILD, and to analyze differences according to weight loss status.

Methods: This was a prospective cohort study. Body composition—assessed by appendicular skeletal muscle index (ASMI) and fat mass (kg)—and BMD at the lumbar spine and femoral regions were measured using dual-energy X-ray absorptiometry (DXA). Functional capacity was evaluated using handgrip strength (HGS) and the 6-minute walking distance (6-MWD). Malnutrition was assessed using the Subjective Global Assessment (SGA). Differences according to weight loss status were analyzed using X2 and Wilcoxon test.

Results: Twenty-six patients were included. During follow-up, 76.9% experienced weight loss and exhibited a significant decline in ASMI, fat mass, and BMD at both anatomical sites (Table 1). In contrast, patients without weight loss showed weight gain; however, a decreasing trend in ASMI was still observed. No significant changes in 6-MWD were detected in either group, whereas HGS decreased in the weight loss group. Overall, malnutrition was highly prevalent among patients with ILD, regardless of body weight changes.

Conclusion: In women with ILD, weight loss was associated with greater longitudinal deterioration in body composition, muscle strength, nutritional status, and BMD, without evident changes in 6-minute walking distance. Weight loss may serve as a clinically relevant marker of nutritional and musculoskeletal decline in this population
