THE EFFECT OF N-ACETYLCYSTEINE SUPPLEMENTATION ON IVF OUTCOMES IN WOMEN WITH ENDOMETRIOSIS

Rationale: Endometriosis negatively affects reproductive outcomes through inflammation and oxidative stress, impairing oocyte quality and IVF success. Antioxidant agents such as N-acetylcysteine (NAC) have been proposed as potential therapeutic options (1). To evaluate the effect of NAC supplementation on oocyte quality and IVF outcomes in women with endometriosis.

Methods: This prospective randomized clinical study included 200 women diagnosed with endometriosis undergoing IVF treatment between 2022 and 2024, as defined in the study protocol  . Patients were randomly divided into two groups: 100 women who received NAC supplementation (600 mg/day for 3 months prior to IVF) and 100 women who did not receive any antioxidant therapy. Baseline characteristics, ovarian response, embryo development, and reproductive outcomes were compared between the groups. 

Results: Baseline characteristics were similar between groups. There were no significant differences in the number of MII oocytes retrieved (6.1±4.5 vs. 5.9±4.3, p=0.72) or day 5 embryos (2.2±1.9 vs. 2.1±1.8, p=0.78). Clinical pregnancy rates per transfer were higher in the NAC group (45.7% vs. 35.5%), although not statistically significant (p=0.22). Live birth rates per clinical pregnancy were comparable (78.1% vs. 77.3%, p=0.94).

Conclusion: NAC supplementation showed a non-significant trend toward improved IVF outcomes in women with endometriosis. This is the first prospective randomized trial to evaluate the NAC effect on IVF results in women with endometriosis.
