Rationale: Despite rapid anthropometric and metabolic improvements after gastric bypass, few studies examine how patients interpret these early changes in daily life. This study assessed biochemical and anthropometric outcomes alongside patients’ lived experiences during the first six months after surgery.

Methods: This is a mixed-methods study with an explanatory sequential approach, conducted at a public university hospital in Brazil. The quantitative phase included 35 patients. Body weight, body mass index (BMI), lipid profile (total cholesterol, LDL-c, HDL-c, triglycerides), and fasting glucose were evaluated two months before and six months after surgery. Comparisons were conducted using paired t-tests or Wilcoxon tests. The qualitative phase involved semi-structured interviews with 11 patients from the quantitative sample at six months post-surgery; narratives were analyzed using a thematic approach and triangulated with clinical data.

Results: Quantitative data revealed statistically significant reductions in body weight, BMI, total cholesterol, LDL-c, triglycerides, and fasting glucose (p < 0.05). Qualitative findings indicated significant gains in quality of life, pain reduction, and improved mobility, while also highlighting challenges regarding dietary adaptation, side effects, and the essential role of family and professional support. The integration of findings revealed a strong alignment between clinical improvements and patients’ subjective perceptions.

Conclusion: Bariatric surgery positively impacts both clinical outcomes and patients' health perceptions. Qualitative insights emphasize the importance of monitoring subjective experiences to ensure comprehensive postoperative care.
