Rationale: Overnutrition drives chronic metabolic inflammation. However, the association between obesity classes, as defined by body mass index (BMI), and inflammation remains debated. We investigated whether Class III obesity linearly exacerbates novel inflammatory indices compared to Class I-II obesity using the multicenter OBREDI-TR cohort.

Methods: This multicenter retrospective analysis included 6,499 of 10,121 adults for whom laboratory data were available from 28 centers. Patients were grouped by BMI: Class I-II (30.0–39.9 kg/m², n=4,555) and Class III (≥40.0 kg/m², n=1,944). We compared anthropometrics, comorbidities, plasma biochemical parameters, and complete blood count-derived indices: neutrophil-lymphocyte ratio (NLR), platelet-lymphocyte ratio (PLR), lymphocyte-monocyte ratio (LMR), systemic immune-inflammation index (SII), and systemic inflammatory response index (SIRI).

 

Results: Class III patients were significantly younger (42.7±13.2 vs. 44.7±14.0 years, p<0.001) with more females (80.6% vs. 68.4%, p<0.001). Despite higher BMI (44.9±4.6 vs. 34.1±2.9 kg/m², p<0.001) and waist circumference (122.4±13.9 vs. 107.5±11.5 cm, p<0.001) in Class III, inflammatory indices showed no significant differences. Surprisingly, hypertension prevalence was lower in the Class III obesity group (49.0% vs. 52.7%, p=0.006), likely due to the female predominance

Conclusion: Progressing from Class I-II to Class III obesity does not linearly exacerbate systemic inflammation. This potential "ceiling effect" aligns with literature indicating that visceral adiposity and specific phenotypes—such as metabolically healthy versus metabolically unhealthy obesity—are more critical determinants of inflammatory and metabolic burden than total BMI alone. Nutritional therapies should prioritize metabolic profiling over simple BMI categorization.
