Rationale: Obesity is a major risk factor for endometrial cancer (EC) and is associated with chronic inflammation, muscle loss, and impaired muscle function, all of which negatively impact clinical outcomes 1. This study aimed to evaluate whether adding physical activity (PA) to a dietary intervention improves skeletal muscle mass (SMM) and strength in EC patients prior to staging surgery (SS).
Methods: This pilot quasi-experimental study included women diagnosed with endometrioid EC scheduled for SS at the National Cancer Institute of Mexico. Participants were assigned to one of two groups: an anti-inflammatory diet alone or an anti-inflammatory diet combined with a structured PA program. Intervention lasted ≈4 weeks, before SS. SMM and handgrip strength were assessed longitudinally at three time points. Preliminary descriptive analyses were conducted.
Results: To date, 19 patients have been recruited; 63% had obesity. Baseline waist circumference was 96.39 ± 13.37 cm and dietary adequacy was 85.4 ± 27.9%. Both groups showed reductions in body weight and waist circumference, with greater tendencies observed in the PA + diet group. Handgrip strength improved in both groups. SMM decreased in both groups, with less reduction in the PA+diet group (Table 1).

Conclusion: These preliminary findings indicate that combined lifestyle interventions are feasible in EC patients prior to SS. Although SMM decreased in both groups, PA was associated with better preservation of muscle mass and handgrip strength compared with diet alone.
