BODY IMAGE AND QUALITY OF LIFE CHANGES DURING WEIGHT LOSS AND WEIGHT MAINTENANCE IN ADULTS WITH OBESITY - RESULTS FROM THE LION STUDY

Rationale: There is limited data on how body image (BI) and quality of life (QoL) relates to weight loss maintenance. This study aimed to investigate the associations between short- and long-term weight changes and changes in BI and QoL.
Methods: Adults aged 18-65 years with a body-mass-index (BMI) of 30.0-39.9 kg/m² were included in the Lifestyle Intervention (LION) Study. Participants followed an 8-week weight loss intervention (WLI) based on a formula diet (800 kcal/day), followed by a 12-month weight maintenance intervention (WMI) and a further 12-month follow-up. Anthropometric data, BI (using the Body Appreciation Scale and Multidimensional Body-Self Relations Questionnaire-Appearance Scale) and QoL (using the Impact of Weight on Quality of Life-Lite) were assessed at each time point. Regression analyses were performed.
Results: In total, 271 adults (64 % female, 45±11 years old, BMI 34.5±2.9 kg/m²) completed the WLI. During eight weeks, body weight decreased significantly by 11.8±3.5 kg while BI and QoL increased (most scores p<0.001). Weight loss was not associated with changes in QoL and BI. Lower baseline scores of BI and QoL were associated with greater changes in the respective scores during WLI (all scores p<0.001). During the 12-month WMI, body weight increased significantly by 2.0±7.7 kg and by 3.8±5.8 kg during further 12 months of follow-up. Most QoL and BI scores further increased significantly during the WMI and showed negative associations with changes in body weight (most scores p<0.001).
Conclusion: BI and QoL increased during the WLI, depending on the respective baseline score. During the WMI, BI and QoL further improved, while the amount of change was negatively associated with body weight change. These results suggest that weight maintenance support is highly relevant in the maintenance of mental health outcomes in people with obesity after weight loss.
