NUTRITIONAL RISK AND CARDIOMETABOLIC BURDEN IN OLDER ADULTS: FINDINGS FROM A NATIONAL COHORT

Rationale: Nutritional risk is common in aging populations and may co-occur with cardiometabolic conditions. This study estimated its prevalence and sociodemographic and cardiometabolic correlates in older adults in Brazil. 

Methods: Cross-sectional analysis of the second wave (2019–2021) of the Brazilian Longitudinal Study of Aging (ELSI-Brazil), including adults ≥50 years. Nutritional risk was defined using a multidimensional index (anthropometry, dietary quality, comorbidities, and physical activity). Associations were estimated using Poisson regression with robust variance, weighted for complex sampling. Waist-to-height ratio (WHtR) was selected among central adiposity measures for the final model. Adjusted prevalence ratios (PR) and 95% confidence intervals (CI) were reported. 

Results: Nutritional risk affected 53.9% (95% CI: 52.4–55.3) of participants. In the final model, higher income (>R$1,250) and ≥9 years of schooling were associated with lower risk (PR = 0.71, 95% CI: 0.65–0.79; PR = 0.83, 95% CI: 0.77–0.90), whereas Black or Brown individuals had higher risk (PR = 1.18, 95% CI: 1.10–1.26). Age was not significantly associated. Greater cardiometabolic burden was independently linked to increased nutritional risk (see Table 1).

Conclusion: Nutritional risk is highly prevalent among older adults and is strongly associated with sociodemographic inequalities and cardiometabolic burden. Population-level monitoring and preventive strategies are needed to address nutritional vulnerability in aging populations.
