Rationale: Surgical stress in patients with gastrointestinal cancer triggers inflammatory responses that may be attenuated by preoperative fasting abbreviation. This study compared whether the addition of whey protein to maltodextrin provides superior benefits in postoperative recovery and nutritional status compared with carbohydrate alone.

Methods: This was a randomized, single-blind clinical trial including adult patients undergoing elective surgery. Participants were allocated into two groups: maltodextrin (25 g) or maltodextrin combined with whey protein, administered 3 hours before anesthesia. Biochemical parameters, complications, and nutritional status (GLIM criteria) were assessed. Fisher’s exact test was used for categorical variables (p<0.05)

Results: A total of 103 patients were included (maltodextrin: n=46; maltodextrin+whey: n=57). Groups were comparable regarding sex, diabetes, smoking status, and surgical characteristics (p>0.3). No difference was observed in time to postoperative diet resumption (p=0.6). A significant difference was found in pre- and postoperative Body Mass Index (BMI) variation (p=0.043), suggesting better body composition preservation. Additionally, a higher proportion of patients with high Prognostic Nutritional Index (PNI) was observed in the maltodextrin+whey group (96.8% vs. 73.7%; p=0.032)

Conclusion: Preoperative fasting abbreviation with whey protein is safe and was associated with improved BMI preservation and better surgical prognosis (PNI), supporting its use in perioperative clinical practice
