MORPHOFUNCTIONAL ASSESSMENT AND POSTOPERATIVE OUTCOME AFTER CYTOREDUCTIVE SURGERY WITH HYPERTHERMIC INTRAPERITONEAL CHEMOTHERAPY (CRS-HIPEC)

Rationale: Nutritional status is believed to be an important factor for postoperative outcome in patients with CRS-HIPEC. Our aim was to assess muscular condition using handgrip strength (HGS) and nutritional ultrasound and to evaluate its involvement for the risk of postoperative complications
Methods: Retrospective study of patients undergoing CRS-HIPEC. Data were collected baseline.1 wk after surgery we evaluated HGS (dynamometer) and rectus femoris ultrasound (methodology and cut-off values from DRECO study) in cross-sectional area (CSA) and Y axis (Yax).
Results: 2/22 (male/female) patients were included.15 ovarian cancer, 4 colorectal, 4 pseudomyxoma peritonei and 1 endometrium. Age 61.2±8 y. BMI 24.1±4 kg/m2, albumin 4,2±0,3 g/dl, prealbumin 25.2±5 mg/dl. SGA: A 45.8% B 25% C 29.2%. 1 wk after surgery: HGS Low strength (P< 20) 50%. In women 20.3± 6 kg. P 30.4± 2.In male patients 39.5±1 kg. P 45±3, Rectus femoris US women: CSA 3.3 ±1.4 cm2. 59.1% probable and 40.9% severe sarcopenia. Yax 9.69±0.3 mm. 31,8 % probable and 40.9% severe sarcopenia: Male patients: CSA 5.5±0.3 cm2 Yax 14.5±0.3 mm. 0% probable or severe sarcopenia. A significantly positive correlation was found between Yax and HGS (r=0.634, p 0.001). Infectious and non-infectious complications were more prevalent if Yax severe sarcopenia:44.4% vs 6.6% (X2 4.48 p 0.03) and 88.8% vs 53.3% (X2 3.65 p <0.05)
Conclusion: Our results show that low HGS, and sarcopenia according to US criteria are common findings after CRS-HIPEC. Postoperative complications were significantly increased in patients with Yax severe sarcopenia. Yax measure shows a positive correlation with HGS.
