Rationale: Parkinson’s Disease (PD) is the second most common neurodegenerative disorder. Its burden has more than doubled in 30 years. More than 80% of patients experience severe gastrointestinal symptoms such as constipation, nausea and vomiting.

Methods: PD is characterized by gut dysbiosis, immune dysfunction, bacterial overgrowth (SIBO) and chronic intestinal inflammation. Gut dysbiosis leads to the reduction in SCFA and the degradation of mucin, increasing intestinal permeability, allowing proinflammatory agents to reach the CNS, promoting neuroinflammation. Significant correlations have been established between gut microbiota alterations and clinical outcomes such as motor symptoms, and cognitive impairment. The gut microbiome secretes neurotransmitters and SCFA that send signals to the brain through the endocrine pathway. Targeting the gut-brain axis with probiotics and prebiotics may help reduce neuro-inflammation, microglia activation​, gut dysbiosis, SIBO​, intestinal hypermeability and help mitigate the oxidative stress and its associated neuronal damages.

Results: Several clinical trials and a meta-analysis demonstrated that probiotics supplementation improved constipation, stool consistency, decreased bloating and abdominal pain severity and improved scores on the UPDRS score and the non-motor symptoms score in PD. Another meta-analysis on 12 RCT including 818 patients with PD confirmed that blends of Lactobacillus and Bifidobacterium improved constipation, increased stool frequency and reduced Parkinson’s UPDRS-III score. Prebiotics may reduce gut dysbiosis and intestinal inflammation. Two open-labeled studies have shown that prebiotics improved gastrointestinal symptoms in PD.

Conclusion: Probiotics and prebiotics supplementation demonstrated clinical efficacy to reduce gastrointestinal symptoms affecting patients' quality of life.
