ASSOCIATION BETWEEN THE PLANETARY HEALTH DIET INDEX AND LIPID PROFILE IN HEMODIALYSIS PATIENTS

Rationale: The EAT-Lancet Commission proposed the Planetary Health Diet (PHD), which prioritizes plant-based foods while limiting the intake of animal-based and refined products to promote health and environmental sustainability. However, its applicability in populations with chronic conditions, such as patients undergoing hemodialysis (HD), remains unclear. The aim of this study was to examine the association between adherence to the PHD Index (PHDI) and lipid profile, as a cardiometabolic marker, in patients undergoing HD.
Methods: This cross-sectional study included 143 patients undergoing hemodialysis (59.4% male; mean age 53.7 ± 14.0 years; BMI 25.7 ± 5.2 kg/m²). Nutritional status was assessed using anthropometric measures (BMI, waist circumference, and neck circumference). Dietary intake was evaluated using a 24-hour recall (24HR), with adjustment for intra-individual variability to estimate usual intake. Adherence to the Planetary Health Diet was assessed using the PHDI, and lipid profile markers were analyzed as cardiometabolic parameters.
Results: Low adherence to the Planetary Health Diet was observed, with a mean PHDI score of 38.5 points (~25.6% of the total score of 150 points). An inverse association was found between PHDI and cardiometabolic risk (p = 0.004).
Conclusion: In this cross-sectional study, patients undergoing HD exhibited low adherence to a sustainable dietary pattern. Higher PHDI scores were associated with a more favorable lipid profile, supporting a potential association between adherence to sustainable diets and reduced cardiometabolic risk in this population.
