PCAGE, A NOVEL METHOD FOR ASSESSING BIOLOGICAL AGING BASED ON BODY SHAPE PRINCIPAL COMPONENTS

Rationale: Aging significantly impacts public health. The need for effective and accessible biomarkers to assess aging has become increasingly urgent.
Methods: Principal component analysis (PCA) was used to summarize eight anthropometric indicators into four body shape principal components (PCs). We used Cox regression models to investigate the associations between body shape PCs and mortality. Based on the four body shape PCs, a novel biological age (BA) metric, PCAge, was developed to assess aging and accelerated aging. Receiver Operating Characteristic (ROC) curves tested PCAge's predictive ability for mortality. Leukocyte telomere length (TL) was further used to validate the association between PCAge and aging.
Results: This study included 13,596 participants from National Health and Nutrition Examination Survey (NHANES) (median age 42 years, 49% female), with a median follow-up duration of 16 years. Cox regression analysis showed that PC1 and PC2 were significantly associated with reduced mortality risk, while PC3 was significantly associated with increased mortality risk. Additionally, PC3 was positively associated with phenotypic age (r= 0.51). PCAge was a strong predictor of mortality (AUC= 0.906), especially in middle-aged participants (AUC= 0.636). Linear regression results showed a significant negative association between PCAge and leukocyte TL (r= -0.45). Based on PCAge, a web calculator was developed to make PCAge more accessible.

Conclusion: Based on body shape PCs, we developed a novel BA metric, PCAge, to assess aging and accelerated aging. PCAge is easily applicable in clinical practice for monitoring and promoting healthier aging.
