Rationale: Non-alcoholic fatty liver disease (NAFLD) is associated with high cardiovascular disease (CVD) prevalence and increased mortality, yet robust risk stratification tools are lacking. The Endothelial Activation and Stress Index (EASI) is a novel composite biomarker reflecting vascular health. This study examined associations between EASI and both CVD prevalence and all-cause mortality in NAFLD patients.

Methods: We conducted a retrospective cohort study of 432 adults (66.9% men, mean age 49±7 years) with NAFLD treated at our institution (May 2017-October 2023). Baseline EASI was quantified from serum lactate dehydrogenase, creatinine, and platelet count. CVD included heart failure, coronary artery disease, angina, and myocardial infarction. Patients were categorized into EASI quartiles (Q1-Q4: 1.72, 3.31, 6.76, 8.12). Logistic regression evaluated cross-sectional CVD associations. Cox proportional hazards models calculated hazard ratios (HRs) for all-cause mortality.

Results: Baseline CVD prevalence was 17.6% (n=76). Highest EASI quartile demonstrated significantly elevated CVD risk (OR=3.41, 95% CI 2.25-5.17). During median 81-month follow-up (IQR 55-96), 49 patients (11.3%) died: 27 cardiovascular, 7 cancer, 15 other causes. Compared to lowest quartile, HRs (95% CI) for mortality were: Q2: 1.09 (0.79-1.17), Q3: 1.42 (1.12-1.69), Q4: 1.79 (1.48-2.10). Restricted cubic splines demonstrated positive linear relationship between EASI and mortality risk.

Conclusion: EASI is cross-sectionally associated with CVD prevalence and independently predicts all-cause mortality in NAFLD. Multi-center prospective studies are warranted to validate these findings.
