Rationale: Protein-energy wasting (PEW) is one of the major risk factors for adverse outcomes in patients with Chronic Kidney Disease (CKD) and those with ESRD who are on dialysis. This study was done to assess the protein intake of ESRD who were on dialysis.
Methods: A cross-sectional descriptive study was carried out from the 30th of November 2023 to the 20th of December 2023 in the dialysis units of the National Hospital of Sri Lanka (NHSL). Patients with ERSD presenting to the Dialysis Unit of NHSL for regular dialysis treatment were included in the study. Patients who were acutely ill were excluded. A detailed dietary assessment was done using a Twenty-four-hour dietary recall with the aid of the Sri Lankan Food Atlas for adults. It was then fully analyzed according to the United States Department of Agriculture (USDA) food data base and Indian food composition tables. Ethical approval was given by the ethical review committee of NHSL.
Results: The total sample size was 131. All the patients were above 18 years of age. The majority (72.5%, n=95) of the patients were males. The mean usual weight was 60.54 kg. The mean total protein taken per day by the ESRD patients was 33.08 grams. The mean protein/ body weight was 0.56 g/kg. The most frequent (86.7%, n=91) reason for poor dietary protein intake was anorexia.
Conclusion: Protein intake per body weight was 0.56 g/kg which was not satisfactory. The protein intake was less than the recommended in 92.3% of the study population. Phenylalanine was the most frequent amino acid detected in the diet while Leucine was the least frequent. There were statistically significant association between protein/body weight and consumption of all essential amino acids. Anorexia and disease related factors were the most common reasons for poor dietary protein intake.
