Rationale: High adherence to the Mediterranean diet reduces the incidence of cardiovascular disease in the general population; however, its impact in people living with HIV (PLWH) remains understudied. The main aim was to investigate whether a nutritional intervention based on the Mediterranean diet can improve lipid profile and dietary adherence. Secondary objectives included assessing changes in anthropometric parameters and arterial stiffness.

Methods: We conducted a 3:1 randomized single-blinded clinical trial comparing a structured nutritional intervention with standard written dietary. We included  PLWH ≥18 years-old, on stable antiretroviral therapy with undetectable viral load for ≥12 months, LDL cholesterol >135 mg/dL, and Mediterranean Diet Adherence Score (MEDAS) <9. Patients under lipid-lowering treatment, with familial hypercholesterolemia, diabetes, inflammatory diseases, hepatitis B and C and pregnancy were excluded. The intervention group received two in-person and two telephone visits with a nutritionist. The control group received generic written dietary advice at baseline.

Results: N=54 participants. Mean age was 40.4 years+8.5 in the intervention group Vs. 42.1 years+7.8 in the control group. At baseline, no individuals had established cardiovascular disease. No differences in lipid profile were observed.

Conclusion: The Mediterranean diet–based intervention improved adherence to the diet and reduced arterial stiffness over 24 weeks in PLWH. The intervention did not significantly modify lipid profile or anthropometric parameters.
