RELEVANCE OF INFLAMMAGING IN PEOPLE WITH ADVANCED CHRONIC KIDNEY DISEASE (ACKD)

Rationale: Inflammaging is a term described in 2000 and refers to premature aging caused by mild but chronic inflammatory processes associated with age and chronic diseases. It is associated with malnutrition and sarcopenia in the general population as well as in people with CKD.
Objective. To analyse the influence of age on morphofunctional assessment parameters (nutrition, body composition and muscle strength) on people with advanced CKD (ACKD)

Methods:  We evaluated 253 people with CKD stages 3b, 4 and 5 who were not dialysis.
Using the ROC curve, we established the best cut-off point of age=76.5 years with sensitivity72% and specificity 62.4%.

We established influence according to age percentiles with morpho-functional assessment: Weight, BMI, Albumin, prealbumin, CRP, body composition by BIVA: Phase angle, Total body water (TBW), Intracellular water (ICW), Extracellular water (ECW), % fat mass, % Fat Free mass, % muscle mass, and Hand grip strength (HGS), VGS and MIS.

Results: 253 CKD people evaluated,153 (60.5%) men, age 71±13.71 years (age percentiles = 63.74 and 82 years), 37.8% DM.

By anova according to age percentiles we found a significant difference (with worse results of the different parameters with age) in: phase angle, Na/k and %cell mass (p0.001), water-extracellular,water-intracellular (p0.001), %fat mass, %muscle mass, % fat free mass  (p0.001), BCMI (p0.001), albumin and prealbumin (p0.001), MIS (p0.016), VGS (p0.04), HGS right 0.010. No significant difference: Total body water%, HB, CRP, lymphocytes, CKD-EPI, arm muscle circumference 0.090.  Performing logistic regression with a cut-off point of 76.5 years, we found the best model with MIS 0.003 and Right HGS 0.023.

Conclusion: CKD involves inflammaging, mainly in older people, and is a facilitating factor for PEW, which justifies the monitoring of nutritional status, body composition and strength to prevent the development of this disease.
