EQUAL ENGAGEMENT: ADHERENCE TO MULTIMODAL PREHABILITATION DOES NOT DIFFER BY SOCIOECONOMIC STATUS

Rationale: Multimodal prehabilitation improves surgical outcomes, but it is unclear if adherence differs by socioeconomic status (SES). This study examines the relationship between SES and patient adherence and compliance in such programs.

Methods: Patients aged ≥16 years and scheduled for elective high-impact surgery were enrolled in a multimodal hospital-based prehabilitation program (2021 – 2024). Adherence was defined as the percentage eligible patients consenting to participate; compliance per patient as percentage of completed sessions/interventions across components: exercise, nutrition (protein and vitamin supplementation), psychological support, smoking cessation, and alcohol cessation. SES was determined using SES-WOA scores based on postal code (Dutch CBS classification)1. Analyses were performed across five SES groups and between low SES and high SES, using chi-square, Kruskal–Wallis, and Wilcoxon tests, and linear and logistic regression adjusted for age, sex, smoking status, and ASA score. A p-value <0.05 was considered statistically significant.

Results: A total of 1,677 patients were included (mean age: 64.0 +/- 12.3 years; 52.8% women). No significant differences in adherence were observed between SES groups, with adherence rates ranging from 71.0% to 74.3%. Mean compliance was 83.0% for exercise and 88.3% for protein supplementation. Compliance for exercise, nutrition, smoking cessation, and alcohol cessation did not significantly differ across SES groups. Only vitamin supplementation was different across individual SES groups (p=0.02).

Conclusion: Adherence and compliance appeared similar across SES groups, suggesting that accessible and well-structured multimodal prehabilitation programs may effectively engage patients regardless of socioeconomic background.
