DISCUSSION NUTRITIONAL STATUS CRITICALY ILL ELDERLY IN SURGERY INTENSIVE CARE UNIT

Rationale: The risk of malnutrition in ICU patients is approximately 40-60%, Taiwan’s elderly population reaching 16% of the total, study aims to investigate the correlation nutritional status and the length of ICU stay in elderly patients admitted to a surgical ICU.
Methods: A retrospective on 2,016 surgical ICU patients from 2011 to 2014, excluding those under 20 or 65 years old and ICU stays exceeding 20 days. A total of 751 elderly patients were included. Data collected included gender, height, weight, BMI, albumin, pre-albumin (≥20 mg/dL and <20 mg/dL), blood urea nitrogen (BUN), serum creatinine, total lymphocyte count, disease severity scores (APACHE II, TISS, GCS), dialysis status, ICU and hospital length of stay, and prognosis. Continuous variables were expressed as mean ± standard deviation, and chi-square tests and T-tests were used to compare groups.
Results: 751 patients, 209 (27.8%) had pre-albumin ≥20 mg/dL, while 542 (72.2%) had <20 mg/dL. Significant differences were observed between groups in ICU stay, total hospitalization days, survival, and mortality (P<0.05). In addition to disease severity, BUN and serum creatinine levels were associated with prognosis.
Conclusion: Elderly patients account for 35% of our surgical ICU population, with numbers expected to rise. Nutritional status, BMI, and renal function at admission can serve as survival evaluation parameters. Future research will focus on whether improving preoperative nutritional status in severely malnourished patients can enhance prognosis.
