M. shuyin1,* on behalf of respiratory nurse

1nutrition department, chimei-medical center, tainan, Taiwan, Province of China

Rationale: patients admitted to the respiratory care center  hope can successfully wean off the respirator. Respirator weaning has many factors.

Methods: medical record review  total of 155 patients who were admitted to the in-patient respiratory care center from October 1, 2020 to July 31, 2019. The patient's basic information is reviewed: gender, age, diagnosis, department, height, admission to intensive care Ward weight, transfer to respiratory care center weight, comorbidities, biochemical values include the time of admission to the care center and transfer out (blood biochemical values: albumin Albumin, pre-albumin , BUN,Cr , SGOP, SGPT, Hb, Na, K, P, among which albumin returns from check-in to transfer out, caloric return reaches 25Kcal /kg, whether the protein reaches 1.2gm/kg, the prognosis is based on the number of days of respirator use, the number of days in ICU, the number of days in RCC, survival and death, grouped by pre-albumin≧20 and <20.

Results: 155 cases  94 (60.6%) were older than 65 years old, and 78 (50.3%) were transferred to medical ICU, of which 84 (54.8%) had pre-albumin ≧20 mg/dl and <20 mg/dl. For 71 patients (45.2%), there was a statistical difference p<0.05 in Albumin, Hb, and Cr between the two groups in terms of comorbidities, the time of transfer to RCC and the time of transfer from RCC. After admission, 25 patients (16.1%) underwent tracheostomy, ICU hospitalization days were 19.3±10.8, RCC hospitalization days were 20.0±6.4, and 15 patients (9.7%) died. There was a statistical difference in prognosis between the two groups P<0.05. Calorie administration >25kcal/kg reaches 88.3%, protein administration >1.2gm/kg reaches 92.9%, pre-albumin and albumin values at the time of RCC admission have a statistical difference of p<0.05 in the 95% confidence interval on the patient's prognosis.

Conclusion: Albumin and pre-albumin levels at the time of RCC admission are valuable prognostic indicators.
