Rationale: Whether standard functional and nutritional markers predict real-time effluent UV toxin load in haemodialysis, and whether this association depends on sex and albumin status, is unknown.

Methods: Retrospective observational study with prospective 12-week follow-up including 1,046 haemodialysis sessions in 61 patients (November 2025–April 2026). Handgrip strength, Short Physical Performance Battery (SPPB) and Malnutrition Inflammation Score (MIS) were available in 48 patients; albumin in 971 sessions (47 patients). The Metabolic Efficiency Score (MES) was derived from the Nikkiso DDM 285 nm sensor. Spearman correlations and subgroup analyses by sex and albumin stratum were performed.

Results: Handgrip was associated with MES overall (ρ = 0.360; n = 300; p < 0.001) and in sessions with albumin ≥3.5 g/dL (ρ = 0.371; n = 276; p < 0.001), but not when albumin < 3.5 g/dL. In men, handgrip was associated with MES (ρ = 0.218; n = 201; p = 0.002). In women, SPPB showed the strongest association (ρ = −0.684; p < 0.001). MIS correlated with albumin (ρ = −0.429; p < 0.001) and prealbumin (ρ = −0.415; p < 0.001). Handgrip declined over 12 weeks (Δ = −1.4 kg; p = 0.002).

Conclusion: The association between functional markers and effluent UV toxin load is sex-specific and was observed only in sessions with adequate albumin status. Hypoalbuminaemia appears to disrupt functional–metabolic coupling. Progressive handgrip decline reinforces its value as a prognostic marker in haemodialysis.
