EFFICACY OF THE HIPROPLADO DIET ON LIVER FUNCTION AND SARCOPENIA IN CIRRHOSIS: A CLINICAL STUDY

Rationale: The adoption of dietary patterns low in fat and sodium and high in protein is recommended for individuals with liver cirrhosis. The aim of this study was to examin the efficiency and safety of a high-protein plant-based diet (Hi-Pro PLADO) in patients with decompensated liver cirrhosis.

Methods: In this single-blind randomized controlled study (ClinicalTrials.gov, NCT0702858), patients were randomized in 3 groups (Group A animal protein, Group B plant protein,  Group C control), receiving individualized energy and protein intake prescriptions. Anthropometric measurements (body weight, height, mid-upper arm circumference, skinfold thickness), hand grip strength and biochemical analyses were performed., Quality of life (QoL) was assessed with the SF-36 questionnaire,.  while dietary intake and adherence were evaluated through 24-hour dietary recalls and a food frequency questionnaire.

Results: One hundred individuals (60 men, mean age 56.7 ± 9.49 years) were enrolled in the study (NGroupA=39, NGroupB=41, and NGroupC=20). Patients in the two intervention groups significantly increased protein intake after 3 months (protein/day = 73.6 ± 13.1 g vs 76.4 ± 11.3 g, p = 0.026), serum protein levels (69.8 ± 10.2 g/L vs 72.4 ± 8.12 g/L, p = 0.015), and handgrip strength (9.44 ± 5.91 kg vs 9.81 ± 4.75 kg, p = 0.0015), while a reduction in γ-GT was observed (94.7 ± 90.1 U/L vs 87.8 ± 83.3 U/L, p = 0.014). At the end of the intervention, Group B presented statistically lower CRP levels (10.3 ± 12.5 mg/L vs 8.06 ± 7.52 mg/L, p=0.001) and triglyceride levels (96.6 ± 37.6 mg/dL vs 89.9 ± 36.10 mg/dL, p = 0.006) compared with Groups A and C.

Conclusion: Individualized diets may improve protein intake, while plant-based protein improves inflammation in decompensated cirrhosis. Functional outcomes, serum proteins, and quality of life remain unchanged.
