ENHANCED LEPTIN REGULATION POST-BARIATRIC SURGERY: THE ROLE OF RESISTANCE TRAINING

Rationale: Leptin plays a central role in energy homeostasis and metabolic regulation and is traditionally associated with adiposity. This study aimed to investigate the determinants of leptin before and after bariatric surgery, focusing on the relative contributions of body composition and metabolic variables.

Methods: A longitudinal study was conducted that included 21 women patients undergoing bariatric surgery who were followed with resistance training. Assessments were performed at pre-surgery and after 12 weeks of resistance training. Variables included leptin, fat mass (FM), total body mass (TBM), fasting glucose, and insulin resistance (HOMA-IR). Bayesian linear regression models were applied at each time point to evaluate independent predictors of leptin. Posterior estimates and 95% credible intervals (CrI) were reported.

Results: At baseline, the multivariate model was significant (p = 0.019), but none of the predictors independently explained leptin levels, suggesting collinearity among adiposity and metabolic variables. After 12 weeks of resistance training, the model remained significant (p = 0.041), and HOMA-IR emerged as a significant predictor of leptin (β = 3.423; 95% CrI: 0.609 to 6.236), whereas fat mass, glucose, and total body mass were not independently associated with leptin.

Conclusion: Following bariatric surgery combined with resistance training, leptin regulation appears to shift from a predominantly adiposity-driven mechanism to a metabolic regulation linked to insulin resistance. These findings suggest a reorganization of leptin physiology in response to weight loss and exercise interventions, highlighting the relevance of metabolic control beyond body composition in post-bariatric patients.
