THE EFFECTS OF ALPHA-LIPOIC ACID SUPPLEMENTATION ON CARDIOVASCULAR RISK FACTORS IN HEMODIALYSIS PATIENTS: A SYSTEMATIC REVIEW AND META-ANALYSIS

Rationale: Cardiovascular disease (CVD) is the leading cause of death in patients with ESRD, including those on hemodialysis (HD) and one suggested intervention is alpha-lipoic acid (ALA) which acts as an antioxidant and improves heart function. This study aimed to investigate the effects of ALA supplementation on CVD risk factors in HD patients through a systematic review and meta-analysis.
Methods: We conducted a search in PubMed, Scopus, Web of Science, and ScienceDirect and included eligible clinical trials published up to September 2024. The quality of the study was assessed using the Cochrane Risk of Bias tool. Statistical analyses were performed using CMA software, with effect estimates derived from fixed or random-effects models. Heterogeneity was evaluated using the I² statistic, and publication bias was assessed via funnel plots, Egger’s regression, and Begg’s test.
Results: The meta-analysis of seven randomized controlled trials (RCTs) demonstrated that ALA significantly reduced C-reactive protein levels (WMD: -2.92 mg/L; 95% CI: -4.12 to -1.72; P < 0.001) and increased hemoglobin (WMD: 1.63 g/dL; 95% CI: 1.35 to 1.90; P < 0.001). However, no significant effects were observed for albumin (WMD: 0.078 g/dL ; 95% CI: -0.02 to 0.17; P = 0.12) or body mass index (WMD: 0.118 kg/m²; 95% CI: -0.66 to 0.89; P = 0.76).
Conclusion: ALA supplementation may improve specific CVD risk factors in HD patients, but further high-quality research is needed to confirm these findings and explore additional outcomes.
