Rationale: Bioimpedance and laboratory markers may relate differently to malnutrition severity in women and men. We explored sex-stratified associations of cell membrane capacitance (CMC), phase angle, and red cell distribution width (RDW) with Global Leadership Initiative on Malnutrition (GLIM) severity.

Methods: In this single-centre prospective cohort study, 95 adults evaluated for pancreatic or upper gastrointestinal surgery underwent nutritional assessment and were classified by GLIM criteria. Sex-stratified analyses were performed in women and men. Group comparisons between not malnourished and malnourished patients used independent-samples t tests. Female-only and male-only ordinal logistic regression models included phase angle, CMC, and RDW.

Results: In women, malnourished patients had lower CMC (997.8 vs 1497.0; p<0.001) and higher RDW (15.63 vs 14.04; p=0.019), while phase angle showed a non-significant trend toward lower values (4.57 vs 5.30; p=0.072). In women, higher CMC was independently associated with lower GLIM severity (p=0.005), and higher RDW with greater GLIM severity (p=0.013). In men, malnourished patients had lower phase angle (4.78 vs 5.96; p<0.001) and lower CMC (1437.6 vs 2349.7; p<0.001), while RDW did not differ significantly (p=0.099). In men, higher CMC remained independently associated with lower GLIM severity (p=0.002), whereas phase angle and RDW were not.

Conclusion: In exploratory sex-stratified analyses, CMC was consistently associated with lower GLIM malnutrition severity in both women and men. RDW showed an additional independent association only in women, while phase angle lost independent significance after adjustment in both sexes.
