Rationale: During a stay in the Intensive Care Unit (ICU), patients experience alterations in quantity and quality of muscle mass, which may lead to functional impairments after discharge.

The aim of this study was to evaluate changes in body composition and functional status over a six-month period following hospital discharge, in patients who survived an ICU stay and received a multidisciplinary intervention.

Methods: A preliminary analysis was conducted within a prospective study of ICU survivors who underwent assessment and follow-up by a multidisciplinary post-ICU clinic. Evaluations were performed at hospital discharge and at 1, 3, and 6 months thereafter.

At each nutritional follow-up visit, fat-free mass (FFM), appendicular skeletal muscle mass (ASMM) and phase angle were assessed using bioelectrical impedance. Muscle function was evaluated (handgrip strength (HGS), and Medical Research Council (MRC) scale). The muscle quality index (MQItotal) was calculated as the ratio of combined HGS to ASMM. The prevalence of ICU-acquired weakness (ICU-AW), defined as an MRC score <48, low muscle mass—defined as an FFM index or ASMM index <15 kg/m² or 5.7 kg/m² in women, and <17 kg/m² or 7.0 kg/m² in men—and low HGS (<27 kg in men and <16 kg in women) was reported. All participants received individualized medical nutrition therapy, with the aim of achieving an intake of 30 kcal/kg/day and 1.5 g/kg/day of protein.

Results: A total of 27 patients were included. At the 6-month follow-up, significant improvements were observed in HGS, FFM, PhA and MQItotal. Additionally, ICU-AW, low muscle mass and low HGS declined during study period (Table 1).

Conclusion: Six months after hospital discharge, ICU survivors exhibited improvements in both the quantity and quality of muscle mass, as well as in functional outcomes.
