SARCOPENIA IN AMBULATORY PATIENTS LIVING WITH TYPE 2 DIABETES (T2DM) IN PERU

Rationale: People with T2DM exhibit higher prevalence of sarcopenia. Peru is experiencing rapid nutritional transitions and increasing prevalence of T2DM. Research examining the incidence of sarcopenia and its association with complications in local settings is limited.
Methods: Retrospective, cross-sectional observational analysis of clinical database from the Cardiometabolic Unit at "Hospital Belén", Lambayeque, Peru. Sarcopenia was assessed according to the European Working Group on Sarcopenia in Older People. Socio-demographic variables were recorded through survey questions. Venous blood samples were collected in the morning following overnight fast
Results: 526 outpatients with T2DM (40 to 80 years); 75.1% (n=395) female. Low muscle strength was identified in 80.4%. Confirmed sarcopenia was highly prevalent, affecting 57,8% (n=304) of the cohort. We found higher prevalence among females (62.3%) compared to male (44.3%). Individuals with confirmed sarcopenia exhibited a significantly higher likelihood of renal complications (OR 1.828, 95% CI: 1.283–2.604; p < 0.001) and 1.571-fold higher odds of neurological complications (OR 2.571 95% CI: 1.801–3.671; p<0.001). 71,7% of sarcopenic individuals had hypertension, corresponding to 2.207-fold higher odds (OR 3.207 95% CI: 2.229–4.616; p < 0.001). Individuals categorized with elevated fat mass exhibited 6.551-fold higher odds of sarcopenia (OR 7.551 95% CI: 4.011–14.218; p<0.001).
Conclusion: Sarcopenia is significantly associated with diabetic complications. We highlight that the distribution of fat mass may be a critical determinant of musculoskeletal health. Diabetic sarcopenia is highly prevalent regardless of age and has emerged as a significant T2DM complication, deserving acknowledgment on par with microvascular and macrovascular complications
