Rationale: Malnutrition in hospitalized older patients remains high, and the first choice of nutritional therapy should be oral nutritional supplements (ONS), which help patients during hospitalization1. Aim: to evaluate the use of ONS in hospitalized older patients.
Methods: A multicentric, retrospective, observational study with wards of older patients from eleven different hospitals in Brazil. Different protocols were applied: assessment of calf circumference (CC) and MAN Short Form, ONS compliance, and food intake, all verified by a dietitian. Stata was used for statistical analysis.
Results: A sample of 679 patients was included: 52% female, 73.4 ±10 years old, and 64% were malnourished or at nutritional risk by the MAN-SF. The main chronic diseases were hypertension(67%), diabetes(37%), and cancer(17%). The main reasons for hospitalization were diagnostic and therapeutic procedures(31%) and heart disease(17%). ONS was used by 45% of the subjects. Patients with infections (88%vs.45%,p=0.015), neurological conditions (59%vs.44%,p=0.03), cancer (57%vs.43%;p=0.004), and without hypertension (58%vs.43%; p=0.030) received significantly more ONS. Patients who received ONS had significantly lower energy expenditure (2037kcal vs 2100kcal, p<0.002) and lower protein requirements (97.5gvs. 108g, p<0.002) than those who did not receive it. Those patients with low BMI (63%<0.001), low CC (58.4%vs.38.8%,p<0.001), and identified as at risk of malnutrition or malnourished by the MAN-SF also received more ONS (p<0.001). However, 46% of the patients who were considered at risk and 32% of the patients considered malnourished didn’t receive ONS.
Conclusion: The prevalence of malnutrition/nutritional risk in hospitalized older patients in Brazil is high. Even at nutritional risk/malnutrition, older patients had underutilized ONS.
