INDIVIDUALIZED MICRONUTRIENT SUPPLEMENTATION IN NURSING HOME RESIDENTS: EFFECTS ON NUTRITIONAL STATUS, METABOLIC MARKERS AND AUTONOMIC REGULATION

Rationale: Nursing home residents are at high risk of hidden malnutrition and micronutrient deficiencies due to ageing, multimorbidity and polypharmacy. Evidence on individualized supplementation remains limited.

Methods: This retrospective controlled study included 130 residents aged 70–82 years matched by disease, medication and age. G1 (n=80) received 16 weeks of individualized micronutrient supplementation including vitamins, minerals, amino acids and omega-3 fatty acids; G2 (n=50) received none. Outcomes included 48-hour heart rate variability (HRV), thyroid-stimulating hormone (TSH), HbA1c, lipids, uric acid, micronutrients, omega-3 index, amino acids and symptoms. Analyses used t-tests or Monte Carlo randomization tests; α=0.05.

Results: Compared with G2, G1 showed significant improvements in HRV: LF/HF ratio 5.17 to 1.98 vs. 5.14 to 5.10, stress index 366 to 219 and pNN50 6.72% to 13.42% (all p<0.001 except uric acid). TSH decreased from 2.51 to 2.00 mIU/L in G1 and remained unchanged in G2. HbA1c decreased from 6.32% to 6.09%, total cholesterol from 256 to 203 mg/dL, LDL cholesterol from 161 to 104 mg/dL and uric acid from 7.26 to 6.08 mg/dL (p=0.021). The omega-3 index increased from 3.2% to 8.6% in G1 and remained low in G2. Vitamin D increased from 33 to 126 nmol/L and selenium from 84 to 118 µg/L; B vitamins, magnesium, zinc and selected amino acids improved significantly. Patient-reported fatigue decreased from 100% to 1%, good sleep quality increased from 56% to 100%, mental performance from 45% to 100% and restlessness decreased from 55% to 19%.

Conclusion: This supplementation was associated with improvements in nutritional status, metabolic markers, autonomic regulation and patient-reported outcomes in nursing home residents. Prospective randomized studies are warranted.
