Rationale: Malnutrition and sarcopenia are common in IBD and negatively influence response to biologics;oral nutritional supplements (ONS) is often poorly tolerated. This study evaluates the efficacy and tolerability of a novel ONS, characterized by low osmolarity, high protein content, a patent blend of polyphenol-rich extracts in patients at risk of or with malnutrition.

Methods: We enrolled 14 IBD patients with moderate-to-severe disease based on clinical and biochemical indices, all receiving biologics. Nutritional assessment was performed at baseline and after 3 months of novel ONS using anthropometry (BMI,MAMC,hand grip strength), BIA, and screening tools (MUST,GLIM,MNA).

Results: At baseline,57.1% of patients had severe malnutrition (GLIM stage 2),64.3% per MUST (≥ 2) and MNA, a median BMI 19±1.3 kg/m², handgrip strength 27±9.1 kg, phase angle 5.5±1, skeletal muscle index (SMI) 8.6 ± 1 kg/m², and mid-arm muscle circumference (MAMC) 23.45±2.7 cm. After 3 months of treatment, nutritional risk scores improved significantly; no patients were classified as malnourished by MNA or MUST≥ 2, and only 8.3% met GLIM stage 2. At the end of the follow-up, body weight increased by 6.5%, with a median BMI 20.4±1.7 (+7.4%,p=0.030), MAMC 25.62±1.3 cm (+9.3%,p=0.015), hand grip strength 32.95±10.67, phase angle 5.7±0.86, SMI 9.17±1.32 kg/m². Biochemical and clinical activity indices also improved. ONS was well tolerated in 85.7% of patients.Two pts drop out:one for worsening of diarrhoea in short bowel syndrome and other for poor adherence.

Conclusion: The novel ONS was effective and safe in improving nutritional status in active IBD pts, supporting the role of a tailored nutritional combined with pharmacological therapy to optimize outcomes.
