VITAMIN D DEFICIENCY AS A NUTRITIONAL RISK FACTOR FOR MORTALITY IN METASTATIC PROSTATE CANCER PATIENTS

Rationale: Vitamin D deficiency in prostate cancer has been correlated with increased tumor aggressiveness and oncologic outcomes but the relation with disease progression or mortality has not been entirely clear.
Methods: A retrospective cohort study was conducted in 149 patients with metastatic prostate cancer at Instituto Nacional de Cancerologia between 2019 and 2020. Serum vitamin D levels were measured and categorized as insufficient or deficient. Kaplan-Meier curves were used to compare survival outcomes. Multivariate model was employed to identify independent predictors of mortality.
Results: The multivariate analysis for mortality showed a significant association between vitamin D levels and greater risk of dead, with a threefold increase in risk when comparing groups (insufficiency vs. deficiency) (OR≈3.2; p=0.016). This result reminds consistent in alternative adjusted models (OR≈2.9; p=0.020). Age, oncologic diagnosis and nutritional diagnosis did not show significant associations. Kaplan-Meier was significant for mortality, not for progression, it occurs earlier in those with vitamin D deficiency.

Conclusion: Overall survival was shorter on vitamin D deficiency group, indicating a sustained effect of vitamin D on mortality. Early assessment and correction of vitamin D levels should be prioritized to improve patient survival.
