PREDICTION OF BARIATRIC SURGERY OUTCOMES USING TRUNK-TO-APPENDAGE FAT MASS RATIO FROM BIOIMPEDANCE ANALYSIS: A RETROSPECTIVE COHORT STUDY

Rationale: Bariatric surgery is an effective intervention for morbid obesity; however, predicting surgical outcomes remains a clinical challenge. Visceral adiposity may influence weight loss outcomes, yet its role is not well established. This study examines whether the trunk-to-appendage body fat mass ratio (BFMTA), derived from bioimpedance analysis (BIA), predicts postoperative weight loss.

Methods: In this retrospective cohort study, BIA data from bariatric patients were analyzed to calculate BFMTA. The primary outcome was excess weight loss (EWL) at 18 months. Multivariable linear regression assessed the relationship between BFMTA and %EWL, adjusting for age, sex, skeletal muscle mass (SMM), and surgical procedure: laparoscopic sleeve gastrectomy (LSG, reference), laparoscopic single-anastomosis duodeno-ileal bypass with sleeve gastrectomy (LSADIS), and Roux-en-Y gastric bypass (RYGB).

Results: Data from 88 patients were analyzed. BFMTA was significantly associated with greater EWL (β = 38.7, p = 0.017). This association remained significant after adjusting for confounders. Among surgical types, LSADIS was associated with significantly higher EWL compared to LSG (β = 24.2, p = 0.0048), while RYGB showed no significant difference (β = 8.6, p = 0.35). Other variables including age, sex, and SMM were not statistically significant.

Conclusion: Higher trunk-to-appendage fat mass ratio (BFMTA) is independently associated with greater excess weight loss 18 months post-bariatric surgery. BFMTA may serve as a useful preoperative predictor of surgical success.
