INFLAMMATORY MARKERS PREDICT QUALITY OF LIFE AND OVERALL SURVIVAL IN PATIENTS WITH PANCREATIC CANCER UNDERGOING CHEMOTHERAPY: A LONGITUDINAL OBSERVATIONAL STUDY

Rationale: Cancer cachexia is characterized by systemic inflammation, negatively affecting quality of life (QoL) and survival rate. This longitudinal observational study aimed to investigate the interrelation between inflammatory markers and QoL from pre-chemotherapy to 9-months post-chemotherapy, as well as overall survival (OS) in patients with pancreatic cancer (PC).
Methods: A total of 54 patients [48.10% male, median age 69.50 (58.75-73.00) years] with PC were examined. Serum IL-6, IL-8, IL-10 and TNF-α were obtained at baseline, 3, 6 and 9 months post-treatment initiation. QoL was evaluated at the same timepoints using the EORTC Core Quality of Life Questionnaire (EORTC QLQ-C30). Changes in cytokines and QoL between timepoints (baseline-3 months, 3-6 months, 6-9 months) were calculated. Associations among inflammatory markers and QoL changes were explored by Spearman’s correlations, while prognostic value of baseline markers for OS was assessed by univariate Cox regression analysis.
Results: At baseline, median EORTC QLQ-C30 score, IL-6, IL-8, IL-10 and TNF-α were 58.33 (50.00-83.33), 2.49 (1.51-4.22), 8.39 (4.47-16.13), 5.55 (3.69-8.78) and 17.81 (12.89-23.03) pg/mL, respectively. From baseline to 3 months post-treatment, ΔIL-6 (r=-0.599, p<0.001), ΔIL-8 (r=-0.333, p=0.036) and ΔIL-10 (r=-0.555, p<0.001) were significantly associated with ΔQoL. ΔIL-10 (r=-0.523, p=0.018) and ΔTNF-α (r=-0.496, p=0.026) correlated with ΔQoL, from 3 to 6 months. No significant correlations were identified during the last quarter. Increased baseline serum IL-8 was found to associate with lower OS (HR=1.0, 95%CI=1.002-1.023, p=0.015).
Conclusion: Changes in cytokines correlated with changes in QoL during chemotherapy in patients with PC, highlighting the need for repeated measures of multiple inflammatory markers. Pre-treatment serum IL-8 can predict mortality risk.
