MALNUTRITION, LOW MUSCLE RESERVES, AND INADEQUATE ENERGY AND PROTEIN INTAKE IN PEOPLE WITH DIABETES RECEIVING HOME-BASED CARE

Rationale: Malnutrition and loss of muscle mass are common in people with diabetes mellitus (DM) and are associated with adverse metabolic outcomes. This study aimed to describe nutritional status, muscle reserves, and energy and protein intake in people with type 2 diabetes mellitus (T2DM) receiving home-based hospitalization.

Methods: A cross-sectional study was conducted in people with DM receiving home-based hospitalization across six Colombian cities between 2021 and 2024. Sociodemographic, anthropometric, and dietary data were collected using a 24-hour dietary recall. Muscle reserves were assessed by physical examination according to ASPEN criteria and by arm circumference (AC) and calf circumference (CC), using sex-specific cutoffs. Nutritional status was classified according to the GLIM criteria. Data were summarized using descriptive statistics.

Results: A total of 140 people with DM were included, of whom 59.3% were women. Malnutrition was identified in 58.6% of participants. Muscle depletion was observed in 65.7%, including 42.9% with moderate depletion and 22.9% with severe depletion, and was more frequent in men than in women (71.9% vs. 61.0%). Moderate or severe AC deficit was identified in 50.0% of participants (women: 42.2%, 21.5 ± 2.4 cm; men: 61.4%, 24.1 ± 2.4 cm), whereas moderate or severe CC deficit was found in 75.7% (women: 75.9%, 25.8 ± 4.1 cm; men: 75.4%, 27.5 ± 4.7 cm). Mean energy and protein intake were 16.8 kcal/kg/day and 0.78 g/kg/day, respectively.

Conclusion: Malnutrition, low muscle reserves, and inadequate energy and protein intake were highly prevalent in this population. These findings support the need for early nutritional screening and timely intervention in people with diabetes receiving home-based care. Analytical studies are needed to examine the relationship of these measures with glycemic control and functional status.
