MEASURED VERSUS PREDICTIVE ENERGY EXPENDITURE IN PATIENTS WITH INFLAMMATORY BOWEL DISEASE: A SYSTEMATIC REVIEW

Rationale: Accurate determination of energy requirements is important for nutritional management in patients with inflammatory bowel disease (IBD). However, predictive equations may not adequately capture disease-related metabolic alterations. This systematic review aimed to summarise evidence on resting energy expenditure (REE), measured using gold-standard methods (indirect calorimetry or doubly labelled water), in adults with IBD.
Methods: A systematic literature search was conducted in PubMed, Scopus, and the Web of Science Core Collection. Eligible studies included those evaluating energy expenditure in individuals with IBD and assessing the performance of predictive equations against reference methods.
Results: A total of ten studies met the inclusion criteria and were incorporated into this systematic review. In most of the included studies (n = 7), no significant differences were observed between measured resting energy expenditure and predictive resting energy expenditure. In contrast, two studies reported that predictive equations significantly overestimated resting metabolic rate (RMR) when compared with reference methods. Conversely, one study found that predictive equations underestimated RMR relative to measurements obtained via indirect calorimetry.
Conclusion: Overall, the findings indicate variability in the accuracy of predictive equations in IBD. Although agreement was observed in most studies, both overestimation and underestimation were reported, highlighting potential limitations in their clinical applicability. Further diagnostic accuracy studies are needed to better evaluate the validity of predictive equations and support more precise nutritional assessment in this population.
