EFFECTIVENESS OF NUTRITIONAL INTERVENTION IN PATIENTS WITH TOTAL GASTRECTOMY FOR CANCER AT VIETNAM NATIONAL CANCER HOSPITAL

Rationale: Total gastrectomy significantly impairs nutritional status, leading to complications and delayed recovery. This study evaluates the effectiveness of an early, specialized nutritional intervention—incorporating hydrolyzed peptides and immune-enhancing nutrients—in improving clinical outcomes for these patients.
Methods: Design: A matched-pair controlled intervention study was conducted on 70 patients (35 per group) from April 2023 to April 2024 at Vietnam National Cancer Hospital.
Intervention: For one month, patients received nutritional counseling, a specialized diet, hydrolyzed peptides, immune-enhancing substances (arginine, glutamine, nucleic acid), and omega-3 fatty acids.
Control: Patients followed standard hospital postoperative diets or family-prepared meals.
Results: Clinical Recovery: The intervention group had a significantly shorter hospital stay (8.5 ± 1.8 days vs. 12.3 ± 4.0 days; p = 0.039) and earlier feeding initiation (48.4 ± 8 hours vs. 143.9 ± 77.0 hours; p < 0.001).
- Body Composition: Weight loss was significantly lower in the intervention group (1.0 ± 1.8 kg) compared to the control (5.3 ± 1.8 kg; p < 0.001). Muscle mass loss was also markedly reduced (0.3 ± 1.2 kg vs. 3.8 ± 2.4 kg; p < 0.001).
- Anemia: The rate of postoperative anemia increased significantly more in the control group (p = 0.006).
Conclusion: Early, comprehensive nutritional intervention effectively preserves body mass, accelerates gastrointestinal recovery, and shortens hospitalization for total gastrectomy patients. Structured nutritional support during both inpatient and outpatient care is essential to optimize treatment effectiveness.
