THE EFFECTIVENESS OF GLUTAMINE SUPPLEMENTATION FOR DIARRHEA SYMPTOM IMPROVEMENT IN HIV/ AIDS PATIENTS: A SYSTEMATIC REVIEW

Rationale: Diarrhea in HIV/AIDS patients is a common and debilitating condition linked to gut mucosal damage and malabsorption. This study addresses the clinically important question of whether glutamine supplementation can improve intestinal health and reduce diarrhea symptoms in this population.

Methods: The systematic review was conducted based on the PRISMA guidelines on March 15, 2025, by two reviewers using the PubMed database and Cochrane Library. The foundations of the research were based on Patients, Intervention, Control, and Outcome (PICO) criteria as mentioned in the following: Patients with HIV/ AIDS and diarrhea symptom, with the intervention of using glutamine supplementation; control with a placebo group. The clinical outcome using urinary excretion of mannitol, diarrhea frequency/ consistency scores or grade of diarrhea. The quality of the included studies was assessed with Cochrane risk-of-bias assessment tools.

Results: Three studies met the systematic review's inclusion criteria. Clinical outcomes were significantly improved using the urinary excretion of mannitol and diarrhea frequency/ consistency scores or grade of diarrhea. Two studies used the urinary excretion of mannitol that reflect the recovery of the intestinal absorptive area, resulting in diarrhea symptom improvement. One study directly used grade of diarrhea to describe the clinical imrpovement.

 

Conclusion: This study showed overall diarrhea symptom improvement better in glutamine or glutamine analogue (alanyl-glutamine) groups compare to the placebo group. Broader-scale of studies on this topic is needed for more reliable results.
