Rationale: Patients on home nutrition support need frequent monitoring, yet many concerns are remotely resolvable; telemedicine may reduce unnecessary in-person visits while preserving safety. We report a large homecare experience within a progressive autonomy model.

Methods: Observational, retrospective analysis of real-world data from the registry of a nutritional homecare company, via a teleconsultation platform, January 2022 to 2025. In this model, telemedicine replaced in-person visits in clinically stable patients trained in EN/PN self-connection. Completed virtual consultations of adult and pediatric patients were included, spanning enteral (EN), oral-supplement, parenteral (PN) and intradialytic parenteral nutrition, dysphagia management, and ketogenic regimens (very-low-protein diets with ketoanalogues). We assessed volume, discipline, mean duration and patient-rated satisfaction (1=poor to 5=excellent); annual mean consultations per patient are reported with 95% confidence intervals.

Results: 4,398 patients generated 38,181 completed virtual consultations, with steady growth (10 in 2022; 649 in 2023; 14,208 in 2024; 23,314 in 2025). Patients were balanced by sex and predominantly adults. Care was multidisciplinary; mean consultations per patient rose over the period. Satisfaction was uniformly high across modules (4.7-4.9). Conclusion: In this large real-world cohort, telemedicine enabled high-volume, brief and highly rated multidisciplinary follow-up across all home nutrition therapies. These findings support telemedicine as a feasible, well-accepted part of a progressive autonomy model to safely reduce unnecessary in-person visits.
