Rationale: Electrolyte disturbances (EDs) are common in hematologic malignancies, but its impact remains unclear. We evaluated associations between severe EDs and in-hospital mortality and length of stay (LOS) in a multicenter Chinese cohort.
Methods: We included 2,801 adult hematologic malignancy patients from 13 Chinese hospitals. Severe EDs were defined as ≥2 serum electrolyte abnormalities. Binary logistic and multivariate linear regression were used, adjusting for Charlson Comorbidity Index, tumor subtype, liver/renal function, anemia, hypoalbuminemia, inflammation, and interaction terms (age, sex).
Results: Of 2,801 patients, 88.1% had ≥1 electrolyte abnormality. After full adjustment, severe EDs were independently associated with increased mortality only in females (OR 5.34, 95% CI 1.86–15.37, p=0.01) and younger patients (<65 years, OR 2.45, 95% CI 1.15–5.22, p=0.01); no association in males or older patients (p for interaction=0.01). For LOS, severe EDs were associated with prolonged stay (β=1.49 days, 95% CI 0.08–2.90, p=0.04), more pronounced in females (β=2.14days, 95% CI 0.54–3.74, p=0.01) and younger patients (β=1.84days, 95% CI 1.12–3.56, p=0.04), but not in males or older patients.

Conclusion: In hospitalized hematologic malignancy patients, the impact of severe EDs on mortality and LOS was modified by age and sex. EDs were independent risk factors for adverse outcomes in younger and female patients, with effect likely attenuated in older and male patients due to higher competing mortality. These findings support demographic-specific electrolyte management.
