GASTROINTESTINAL INTEGRITY AND INFLAMMATORY TONE IN BOTH PEDIATRIC AND ADULT IN-VITRO MODELS USING PLANT BASED ENTERAL FORMULAS

Rationale: Pea protein plant-based enteral formulas (PPPBF) have demonstrated adherence and improved gastrointestinal (GI) tolerance in pediatrics and adults. This experiment was conducted to test the effect of formulas containing yellow pea and fermentable fiber on the protective, positive effect of colonic fermentation in an in vitro model.

Methods: We used an in vitro co-culture model of intestinal epithelial-like cells (Caco-2) and human monocytes/macrophages (THP1-Blue™ cells, InvivoGen). GI integrity was measured by increased transepithelial electrical resistance (TEER), and inflammatory tone was assessed by cytokine & chemokine expression. Two pediatric PPPBFs (P1,P2) were tested in 5 healthy pediatric donor fecal samples (A, B, E, G, I) and 2 adult PPPBF (A1,A2) in 5 healthy adult donors (K, L, N, P, Q). A negative control (NC) was included. Statistics used GraphPad Prism 10.2.0.

Results: Across pediatric and adult donors, no significant effects on TEER were observed. However, individual differences were observed in pediatric donor I (treated with P2) and in adult donor L (treated with Adult A1 and A2). Across donors, pediatric P1 and P2 significantly decreased the secretion of TNF-α, CXCL10, and MCP-1 (p<0.05) and significantly increased IL-10 (p<0.05). Similarly, Adult A1 and A2 decreased CXCL-10 in 1/5 donors, MCP-1 in 2/5 donors, and significantly increased the secretion of the anti-inflammatory cytokines IL-6 (3/5) and IL-10 (5/5) (p<0.05).

Conclusion: Individual sample differences were observed with impact on gut integrity using the in-vitro model.  Anti-inflammatory impacts were observed with both Pediatric and Adult PPPF.  A model using IBD samples should be further explored to assess significance as compared to healthy donors.
