BRAIN OXYGENATION AND COGNITIVE PERFORMANCE AFTER SLEEP DEPRIVATION: ACUTE EFFECTS OF CREATINE-GAA AND CAFFEINE SUPPLEMENTATION

Rationale: About one third of adult population is affected by sleep deprivation (SD) throughout a lifespan. Consequently, SD disrupts energy homeostasis, causing systemic inflammation and impaired cognitive function.
Methods: A total of eighteen healthy participants (10 females, aged 23.1 ± 2.6 years, body mass index 21.5 ± 2.6 kg/m2) volunteered in this non-superiority randomized controlled trial. Outcomes included change in cerebral oxygen saturation (StO2) and total hemoglobin (tHb) using functional near-infrared spectroscopy while performing the Attention Network Test at baseline and following a 24-hour SD. Subjects were randomly assigned to receive a single dose of creatine–guanidinoacetate (GAA), caffeine, or placebo supplementation in a parallel-group design after SD. All time × group interaction effects were analyzed employing Generalized Linear Mixed Models with treatment and time as fixed effects using the {emmeans} R package.
Results: Following SD, GAA significantly boosted resting prefrontal StO2 (β= -0.188, +20.7; P= 0.008) while caffeine significantly boosted resting occipital tHb (β= -0.087, +9.1%; P< 0.001). During cognitive testing, the GAA group exhibited a reduction in occipital tHb (β= 0.049, -4.9%; P= 0.010) alongside increase in temporal StO2 (β= -0.110, +11.6%; P= 0.045); the caffeine group exhibited elevations in occipital tHb (β= -0.066, +6.9; P= 0.002), while experiencing reductions in both temporal StO2 (β= 0.158, -14.6%; P= 0.009) and tHb (β= 0.086, -8.3%; P= 0.028). Despite hemodynamic changes, no significant changes were observed of selected cognitive measures.
Conclusion: Supplemental caffeine and creatine-GAA may stimulate selective neurovascular adaptations in response to mental demands after SD in healthy men and women.
