“MANAGING UNCONTROLLED DIABETES BY EARLY INITIATION OF INSULIN AND ADOPTING HEALTHY LIFESTYLE”

Rationale: The prevalence of diabetes has been increasing and new regimes have raised hopes that diabetes can be controlled by early initiation of insulin, adopting healthy lifestyle.  42 years old Obese, hypertensive male with sedentary lifestyle and family history of diabetes was recently diagnosed to have Type II Diabetes. The objective was to manage Uncontrolled Diabetes, prevent diabetic ketoacidosis by early initiation of insulin and lifestyle management.

Methods: The patient was assessed nutritionally using the modified subjective global assessment tool and the dietary recall method was used to assess the daily intake. The dietary management also included the regular check of biochemical parameters and anthropometric measurements.

Results: During first 2 months, patient was started on Ryzodeg insulin, antidiabetic oral medication along with 1400kcal/ 65 gm protein diet with regular walk. Insulin was gradually tapered off and completely stopped by 2 months while Diet and exercise along with oral anti diabetic drugs with decreased dosage were continued. Biochemical parameters improved significantly with improved  HbA1C, BMI with better nutritional status.

Conclusion: The rationale for early initiation of insulin is based on overcoming glucotoxic effects of hyperglycaemia, preserving beta cell mass and function, improving insulin sensitivity with anti-inflammatory and antioxidant properties that may help protect against endothelial dysfunction and damage. Healthy lifestyle resulting in healthy weight loss is linked to lower the risk of complications.
