PATIENT-REPORTED IMPROVEMENT IN MUCOSAL DRYNESS, GASTROINTESTINAL SYMPTOMS AND JOINT COMPLAINTS DURING NUTRITIONAL SUPPLEMENTATION

Rationale: Therapy-associated mucosal dryness, gastrointestinal symptoms and joint complaints may impair oral intake, hydration and vitality in patients with cancer.

Methods: This prospective physician-documented observational case series was conducted in Germany. A total of 75 adults (including 47 with an underlying malignancy) received a dietary supplement containing bromelain, papain, lentil extract, biotin, vitamin C and selenium. Symptom scores ranging from 0 (none) to 5 (extreme) were recorded and assessed for mucosal dryness (MD), gastrointestinal symptoms (GI) and joint complaints (JC) at baseline and at weeks 4, 8 and 12. Responder: ≥1-point improvement (minimal detectable difference).

Results: 71 documented cases were evaluable for responder analysis. Responder rates were 86% (MD), 46% (GI) and 41% (JC). Overall, 87% of users reported first noticeable improvement in ≥1 domain within 4 weeks. Scores for MD, GI and JC were significantly reduced after 4, 8, and 12 weeks. MD showed sustained monotonic improvement over time (Page’s trend test p=0.0136). Overall quality of life improved in 66% and was more frequent in MD responders compared to non-responders (75% vs 25%, p=0.0013). No serious adverse events were reported; one discontinuation occurred due to suspected pineapple allergy.

Conclusion: In this real-world cohort, nutritional supplementation was associated with early and sustained improvements in patient-reported mucosal dryness, along with a signal for reduced GI symptom burden and improved vitality. These findings are hypothesis-generating and highlight the need for controlled studies with nutrition-related endpoints, including oral food and fluid intake, nutritional risk, treatment tolerability, and quality of life.
